With only 9.18% flat terrain (rank 3/390 for mountainousness), the course is almost continuously ascending or descending at steep gradient. Average uphill gradient ranks 40th of 390 for steepness; average downhill gradient ranks 339th of 390 for gentleness (meaning the descents are also among the steeper in the database). The course runs through technical singletrack, exposed ridgelines, forested trails, and high pre-Alpine terrain above the Lake Lugano basin. Both ascent and descent skill are required in equal measure.

Public transport is possible -- Lugano central station then local bus or taxi to Tesserete -- but a car is generally easier for gear transport. Accommodation is strong around Lugano; book early as the race weekend can fill. The K120 starts in June (2026 edition June 12 -- passed; next edition expected June 2027). The valley start in Tesserete is warm (21-24 degrees C June daytime) but the Denti della Vecchia ridge sections (Monte Bar, Gazzirola on the Switzerland-Italy border) are significantly colder -- layers and rain gear are essential for the 120km distance.
